Role Overview

We are seeking a highly motivated and experienced PCN Clinical Pharmacist to join our team on a full-time, remote basis. You will work collaboratively with GPs, nurses, and other healthcare professionals to provide expert medicines management, structured medication reviews (SMRs), and support long-term condition management. This role offers the flexibility of remote working while making a significant impact on patient care within PCNs.

Key Responsibilities

  • Conduct structured medication reviews (SMRs) for patients with polypharmacy, long-term conditions, or those at risk of medication-related issues.
  • Support medicines optimisation and ensure safe, evidence-based prescribing across the PCN.
  • Provide clinical advice and support to GPs, nurses, and other healthcare professionals on medication-related issues.
  • Assist in the management of repeat prescribing systems, medication reconciliation, and deprescribing where necessary.
  • Support patients with chronic disease management, including diabetes, cardiovascular disease, and respiratory conditions.
  • Review and action clinical queries related to medications, including hospital discharge summaries and outpatient prescriptions.
  • Contribute to antimicrobial stewardship and support best practice in prescribing antibiotics.
  • Provide medicines-related education and training to staff within the PCN.
  • Work within local and national guidelines, including NICE guidance, to ensure compliance and best practices.
  • Assist with audit, quality improvement projects, and performance monitoring to enhance medication safety and efficiency.

Essential Requirements

  • GPhC-registered pharmacist with the right to work in the UK.
  • Minimum 6 months of primary care or PCN experience
  • Experience conducting structured medication reviews (SMRs)
  • Excellent clinical decision-making and communication skills.
  • Ability to work independently in a remote setting while collaborating with multidisciplinary teams.

Desirable Qualifications & Skills

  • Experience in frailty, care home medication reviews, or mental health medication management.
  • Understanding of QOF, IIF, and DES targets relevant to PCNs.
  • Strong organisational and problem-solving skills.
