Role Overview
We are seeking a highly motivated and experienced PCN Clinical Pharmacist to join our team on a full-time, remote basis. You will work collaboratively with GPs, nurses, and other healthcare professionals to provide expert medicines management, structured medication reviews (SMRs), and support long-term condition management. This role offers the flexibility of remote working while making a significant impact on patient care within PCNs.
Key Responsibilities
- Conduct structured medication reviews (SMRs) for patients with polypharmacy, long-term conditions, or those at risk of medication-related issues.
- Support medicines optimisation and ensure safe, evidence-based prescribing across the PCN.
- Provide clinical advice and support to GPs, nurses, and other healthcare professionals on medication-related issues.
- Assist in the management of repeat prescribing systems, medication reconciliation, and deprescribing where necessary.
- Support patients with chronic disease management, including diabetes, cardiovascular disease, and respiratory conditions.
- Review and action clinical queries related to medications, including hospital discharge summaries and outpatient prescriptions.
- Contribute to antimicrobial stewardship and support best practice in prescribing antibiotics.
- Provide medicines-related education and training to staff within the PCN.
- Work within local and national guidelines, including NICE guidance, to ensure compliance and best practices.
- Assist with audit, quality improvement projects, and performance monitoring to enhance medication safety and efficiency.
Essential Requirements

GPhC-registered pharmacist with the right to work in the UK.
- Minimum 6 months of primary care or PCN experience
- Experience conducting structured medication reviews (SMRs)
- Excellent clinical decision-making and communication skills.
- Ability to work independently in a remote setting while collaborating with multidisciplinary teams.
Desirable Qualifications & Skills
- Experience in frailty, care home medication reviews, or mental health medication management.
- Understanding of QOF, IIF, and DES targets relevant to PCNs.
- Strong organisational and problem-solving skills.
