We’re a small, patient-centered team that values consistent follow-through, strong documentation, and proactive communication.<br></p>The Role<p>We are hiring a <strong>part-time RN Care Manager</strong> for <strong>10–20 hours per week</strong> in a hybrid position. After training, the role will include <strong>approximately 1 clinic day/week in Tuscaloosa</strong>, with the remaining work completed remotely from home.</p><p><br></p><p>This position is ideal for someone who is <strong>responsible, organized, self-directed</strong>, and comfortable working independently in a healthcare coordination environment.<br></p>Schedule Expectations<ul><li><strong>Clinic day (Tuscaloosa):</strong></li><li>At least <strong>8:30 AM–12:00 PM (Mon–Th)</strong> <em>or</em> <strong>8:30 AM–5:00 PM </strong><strong>(Mon–Th)</strong> on clinic days</li><li><strong>Remote schedule:</strong> Flexible, but ideally able to work <strong>1–2 hours daily</strong></li><li><strong>Weekly hours target:</strong> <strong>10–20 hours/week<br></strong></li></ul>Key Responsibilities<ul><li>Conduct CCM and RPM patient outreach and follow-up calls</li><li>Support patient enrollment and onboarding into care management programs</li><li>Document calls and care plan updates thoroughly and on time</li><li>Track tasks, documentation, and follow-ups consistently</li><li>Communicate clearly with the supervising clinician (you’ll report directly to the owner)</li><li>Work efficiently and independently in an autonomous setting</li><li>Maintain organized reporting and workflows using Excel and shared documents<br></li></ul>Qualifications<ul><li><strong>Active Alabama RN license (required)</strong></li><li>Strong communication skills and reliable responsiveness</li><li>Comfortable working independently with minimal supervision</li><li>Strong documentation habits and attention to detail</li><li>Proficiency with <strong>Excel</strong> and either <strong>Google Workspace and/or Microsoft Office</strong></li><li>Experience in CCM, RPM, primary care, care coordination, or case management is a plus (not required)<br></li></ul>What We’re Looking For<p>This role requires someone who can:</p><p> ✅ manage time independently</p><p> ✅ follow through consistently</p><p> ✅ document accurately</p><p> ✅ communicate proactively<br></p>How to Apply<p>Please apply with your resume and include:</p><ul><li>Your weekly availability</li><li>Whether you can commit to <strong>1 clinic day/week</strong> after training</li><li>Any prior care coordination / CCM / RPM experience</li></ul><p><br></p>
