About the position

Responsibilities

• Provides social casework to individuals and families receiving nursing, therapy, or other services from the contracted agency.
• Prepares social histories according to specific guidelines to augment existing services or guide changes in service levels.
• Assesses family relationships, the meaning of illness, cultural attitudes, and the financial impact of illness on families.
• Evaluates a family's financial situation and refers them to agencies for financial assistance when necessary.
• Refers patients and families to community agencies with appropriate follow-up.

Requirements

• Graduate of an accredited Social Work program.
• Current licensure and eligibility as a Medical Social Worker.
• Current BLS Certification.
• Minimum one year prior experience in a medical setting utilizing a team approach to treatment.
• Minimum one year prior home health experience preferred.
