Overview
CareChoice Homecare is seeking an experienced per diem Home Health Nurse (RN or LPN, per agency needs) to provide skilled nursing care to patients in their homes. The ideal candidate has prior home health experience and is confident working independently in the field while collaborating closely with the interdisciplinary team to support safe, timely, and patient-centered care.
*This is a field nurse position and requires you to be IN PERSON (not remote)
Key Responsibilities
Must have specialty and experience in IV Infusion, would care, and wound vac.
Provide skilled nursing visits in the home, including comprehensive patient assessments, vital signs, and focused physical exams.
Develop, implement, and update individualized plans of care in collaboration with the physician and home health team.capitalhealthhomecare.
Administer medications and treatments, perform wound care, and monitor for changes in condition, intervening and escalating appropriately.
Educate patients and caregivers on disease processes, medications, diet, safety, and self-management to promote independence and prevent rehospitalization.
Document all care, assessments, and patient/caregiver interactions accurately and timely in the electronic medical record, in accordance with home health regulations and agency policies.
Communicate effectively with physicians, therapists, aides, and office staff to coordinate services and ensure continuity of care.
Participate in start-of-care, resumption of care, recertification, and discharge visits as assigned.
Follow all infection control, safety, and regulatory standards for home health practice.
Schedule and Expectations
Per diem, field-based role with flexible scheduling; shifts and visit blocks are offered based on agency needs and your availability.
May include weekdays, weekends, and occasional holidays by mutual agreement
Reliable, timely response to visit offers and strong time management for completing assigned visits and documentation.
Paid per visit $35-85 depending on type of visit
Required Qualifications
Must have specialty and experience in IV Infusion, would care, and wound vac.
Knowledge with home health infusions and wound care / wound vac management
Current, unrestricted RN or LPN license in the state of practice.
Minimum 1 year of recent home health nursing experience (RN or LPN) required; additional acute care or med-surg background preferred.
Current BLS/CPR certification for healthcare providers.
Valid driver’s license, reliable automobile, and current auto insurance for daily field travel.
Proficiency with electronic medical records and ability to complete documentation within required timeframes.
Strong assessment, critical thinking, and independent decision-making skills appropriate for the home setting.
Excellent communication, teaching, and customer service skills when working with patients, families, and referral partners.
Preferred Qualifications
Prior experience with start-of-care and recertification visits.
