Conduct assessments and evaluations of individuals struggling with behavioral health challenges to identify their needs and develop personalized care plans.
Collaborate with members, providers, and other healthcare professionals to coordinate and implement evidence-based treatment plans.
Monitor and track progress of individuals in the care plan, providing ongoing support and making adjustments as needed.
Educate members and their families on available resources and support services to help them manage their behavioral health challenges.
Act as a liaison between members and their healthcare providers to ensure effective communication and coordination of care.
Utilize clinical knowledge and expertise to identify potential barriers to treatment and develop solutions to overcome them.
Maintain accurate and up-to-date documentation of all interactions and interventions with members and providers.
Adhere to all ethical and legal standards in the delivery of care and maintenance of confidential information.
Stay current on industry trends, treatment modalities, and best practices in behavioral health to continuously improve the quality of care provided.
