Qualifications:
  • Bachelor of Arts Degree in Social Work or related field
  • Minimum of one (1) year social work experience in a health care setting
  • Bilingual in English and Spanish
Responsibilities:
  • Assess the psychosocial functioning of the patient and family system
  • Complete bereavement assessments and provide initial and ongoing bereavement support via phone contacts and home visits as needed
  • Maintain bereavement department compliance and quality assurance to meet Joint Commission standards
  • Assist in planning, monitoring and evaluating bereavement programs (English/Spanish)
  • Provide bereavement education to the community
  • Train new hires and interns on bereavement program (i.e. agency protocols and documentation)
  • Create and maintain patient charts and documentation within agencies electronic medical records system
  • Provide interpretation and translation services as needed
  • Assist Director of Social Services and MSWs with scheduling, data entry, and report analysis.
  • Coordinate bereavement department mailings condolence cards, welcome newsletters, quarterly newsletter mailings anniversary newsletters, additional grief resources and no contact letter, as needed.
  • Assist in planning annual memorial service
